Mustard Leadership Parent Consent Form

Dear Parent/Guardian, 
Mustard Leadership Australia (Mustard) is an organisation dedicated to empowering and equipping high school students across Australia to run Christian Lunchtime Groups in schools. Our vision is to see lives transformed by Jesus and we are passionate about supporting young people, such as your child, as they grow in faith and leadership. We are committed to creating a safe environment for every student, prioritising their wellbeing and reducing the risk of possible harm. All Mustard staff and volunteers hold a current Working with Children Check and have completed thorough reference checks. Interaction with students may occur through email, text, phone calls, video calls, or face-to-face meetings. Most communications with students occur in group settings, although there are times when students may participate in individualised mentoring. We aim to ensure that at least two adults are present during video or in-person meetings. When this isn’t possible, we take steps to ensure accountability—such as recording the session, meeting in public settings and keeping detailed notes stored securely. If you choose not to provide consent for Mustard to contact your child directly, we will not communicate with your child outside the school environment. Please be aware that we may still interact with your child as part of a school activity.

From time to time, we may take photographs or record video of students during activities for use in Mustard’s marketing and communications. These materials help us share the impact of the ministry with schools, churches, and supporters. 

We’d love to stay connected with you as your child participates in Mustard activities. Keeping you informed about students’ involvement, achievements, and upcoming events, is important to us. These opportunities not only celebrate your student’s growth but also give you a chance to connect with the wider Mustard community. Therefore you will automatically be linked in with our communication system, which you can choose to opt out of at any time by simply contacting info@mustard.org.au.

I acknowledge the Child Safe practices that Mustard has outlined above to safeguard the protection of students.
I give permission for Mustard’s staff and volunteers to contact my child for the purposes of training and encouraging them to live out their faith at school (which may occur outside of normal school hours).
I give media permission for photos and video involving my child to be used in Mustard’s marketing material and promotion.
